Non-communicable diseases pose significant challenges for elderly Malaysians, often requiring frequent hospital visits. This paper proposes an IoT-based healthcare software solution to improve NCD management and reduce healthcare costs. The software utilizes wearable sensors for vital sign monitoring, provides medication reminders, enables teleconsultation, and triggers emergency alerts. Developed using Agile methodology, the software aims to enhance medication adherence, minimize hospital visits, and facilitate timely medical intervention, ultimately improving the quality of life for elderly NCD patients in Malaysia.
